Sign up here or log in here using your Google account, Microsoft account, or any email.If you sign up with an email, we send a 6-digit code to your inbox. Enter it in the same browser, then choose your name and password. The code lasts 15 minutes. If it doesn’t arrive, check your spam folder or request a new code once the timer runs out.

Create a new form

On your Home page, click New on the right, next to Invite members. Then, select Form.

In a new organization with no workspaces, the New button doesn’t show on Home yet. First, create a workspace with the + next to Workspaces in the left sidebar. After that, you can click New and select Form.

Next, choose how to get started: a Blank form, Create with AI, or Connect with 3rd party apps like Airtable, Notion, HubSpot, and more. You can also click Import existing forms to import a form, convert a PDF, or start from a Template.If you’re unsure about which to use, choose Blank form. You can always connect to an integration later.

Customize the design

Click Theme to pick from one of the pre-made themes or customize your own.

Add questions or fields

Click or drag and drop your desired field types from the left-hand panel. Choose between over 50 different types.Then, enter the question or text. Click its settings icon to customize it in the right-hand panel.

Publish and share

Preview your form to test it. Once everything’s good to go, press Publish.The first time you publish a form, a Publish form popover asks where its responses should go.Open the dropdown under Responses go to and pick an option, then click Publish:

• New database - responses are sent to a new Zite Database that you can build on with tables, custom fields, and views.
• Existing database - responses are sent to a new table in an existing workspace.
• Just this form - no database is attached. Responses are saved to the form’s Results.

If your account’s email isn’t verified yet, you’ll see Verify your email to publish. Open the link in the verification email, or click Resend link to get a new one. Then, go back to the editor and your form will publish on its own.

That’s it! Share your form link anywhere or embed your form on any website. To view form submissions, head over to Results in the top menu.
