Overview

With Fillout, you can easily create and customize your own free quiz that lets you:
  • Automatically score submissions
  • Show answers after each question or list them at the end
  • Sends a notification whenever someone completes your quiz

How to create a quiz in Fillout

Open form settings

Add your questions

Drag and drop the quiz questions you’d like to use from the left-hand panel to your form. Select from dozens of different types.

Set up scoring

Assign score values to each question. Fillout automatically tallies the scores for you. Click a question followed by .Screenshot 2025-10-15 at 12.50.41 PM.pngPut a ✓ beside the correct answer. You can choose multiple correct answers, if applicable, and even define a score value for each answer (see weighted scores below).Set up scoringAdding custom feedbackYou can also display a custom message to respondents based on their answers. Customize the design

Now, the fun part! Customize the look and feel of your quiz with Fillout’s Form designer. Simply, click Theme. For more options, scroll down and open the Advanced Designer.Screenshot 2025-10-15 at 12.48.37 PM.png

Publish and share

Once your quiz is complete, click Publish on the upper right corner and share the link or embed it anywhere.You can also enable email notifications for whenever someone fills out your quiz. The respondent’s score will be included in the email.

Review and analyze results

Fillout provides real-time reporting and analytics on your quiz results. You can view individual responses or analyze trends and patterns in the Results page of your quiz.

Advanced quiz features

Add weighted scores Typically, you’ll assign a score of 1 for correct answers and 0 for incorrect ones, but you can also use weighted scores to give certain questions or answer choices more impact. Simply enter the desired score for each answer choice to apply weighted scoring. Screenshot 2026 04 28 At 5 49 48 PM Display the respondent’s current score You can show the respondent’s score at any point during the quiz. To reference, simply type @ anywhere and click followed by Score. Accessing the respondent’s score in the quiz Show custom ending messages or pages Display different ending messages depending on a respondent’s score by using hide/show logic. To display different ending pages, set up page logic. Drag your mouse across the relevant fields. On the right-hand pane, Set conditional logic.
